Liability Waivers

Every participant in our programs, including family members and personal care assistants, will need to fill out our liability waivers. We keep waivers on file, so you just need to fill out them out just one time per calendar year.

Because we programs both with and without the partnership with the Department of Conservation of Recreation's Universal Access Program, we need 2 waivers from all participants.
